Complete the entry form with your first and last name, email address, mailing address (no P.O. boxes), birthdate, gender, and daytime and evening telephone numbers. You must complete the entry form in its entirety for the Giveaway entry to be eligible. Entries will be deemed made by the authorized account holder of the email address submitted at time of entry.
